When someone in your family falls ill, you should not have to face uncertainty alone.
Tell us where the system supported you and where it left you on your own. Your experiences help us build a picture of what needs to change — so that difficult moments bring more clarity, fairness and humanity.
Share your experienceAn initiative of Future Slovakia Forum, a civic association — REFORMA 3.3 was developed by its healthcare team.
Expert data alone are not enough. We need real stories.
We are working with experts to change healthcare. We have the analytical data. What we are missing is what statistics cannot show: your personal experience.
You help identify systemic failures
Your stories reveal problems that do not appear in the numbers — waiting times, communication and the human side of care. These are the things that need to change.
Your voice matters
Your experiences help those working on change. Real stories from citizens are the strongest argument for better healthcare.
You help us measure quality
You rate doctors, nurses, communication and compassion on a scale from 1 to 5. This is not a representative survey, but your ratings help us find where care works and where it fails.
Three steps. No registration.
The form consists of six short screens grouped into three stages. You select from options, rate your experience using scales, and describe your story in your own words. The form is currently available in Slovak only — if you prefer English, write to us by email.
Give your consent
Brief informed consent. No name, no registration. Just confirmation that you understand how we will use your data.
Describe your experience
Who the patient was, where it happened, and what type of care was involved. Then describe what happened in your own words.
Rate and submit
Rate doctors, nurses, communication and compassion on a scale from 1 to 5. Review the summary and submit.
We will use your experience only in aggregated form. Please do not include names or other details that could identify anyone.
The form is in Slovak. You can write to us in English.
If the Slovak form is not convenient for you, send your experience by email to reforma@healthy-future.sk with the subject “Patient Survey: …” — for example “Patient Survey: hospital stay in Košice”. You can write as the patient yourself or on behalf of a family member or friend. It helps us most if your email covers the same points as the form:
- Who you are in the storyThe patient yourself, a family member (spouse or partner, parent, child, sibling) or a friend or acquaintance.
- What kind of care it wasAn outpatient visit or treatment, a hospital stay or something else — and roughly how long it lasted: one day, a few days, a week or two, a month or more, or long-term care.
- Where it happenedThe name of the facility if you know it — otherwise just the town and the type of facility: hospital, outpatient clinic, polyclinic, emergency department, home-care agency, ambulance service or rehabilitation centre. If you know, also who runs it: the state, a self-governing region, a private operator or a church.
- What happenedIn your own words, as you remember it — what went wrong, or what went well. It does not have to be long or perfect. If you can, add when it happened (the month and year are enough), how long you waited for treatment or admission, whether it was a one-off or a repeated experience, and whether anyone explained why things happened as they did.
- How you rate the careOn a scale from 1 (worst) to 5 (best): doctors · nurses and nursing staff · staff overall · communication and information · humanity and compassion. A short comment on any of them is welcome.
- Anything elseAnything we have not asked about that you consider important. Positive experiences matter just as much as negative ones.
